• All employees, and all contracted individuals of Upper Adams School District must obtain a Pennsylvania State Police Criminal record Check, a Pennsylvania Child Abuse History Clearance, and an FBI Federal Criminal History Clearance (fingerprint background check) prior to working in the district that are within one (1) year of employment. As of January 1, 2015, all employees in school districts must obtain new clearances every five (5) years. 

    In addition to clearance requirements, Act 24 of 2011 of the Pennsylvania School Code requires that all school employees complete and return to their school administrator an Arrest/Conviction Report and Certification Form (PDE Form 6004) within 72 hours of any arrest or conviction of an offense listed in Section 111(e) (see form). If an employee refuses to submit the form, Act 24 requires that the School Administrator submit the employee to a new background check under Section 111.

    Tips and Instructions

    • Do not click the yellow "New Record Check" this is for volunteers only
    • Click "Individual Request"
    • Enter the information reason as "Employment", and enter all information requested
    • Click "Enter This Request"
    • Click "Finished" and review all information for accuracy and click "Submit"
    • Click "Check Out" and enter payment information
    • If your application processes immediately, there will be a control number highlighted in blue, and the statement "no record". You must click on the control number to bring up the OFFICIAL certification.  Click the blue "Certification Form" link to print the certificate with the state seal.
    • Keep a copy for your records, and send the certificate to Tina Fair, Human Resources Coordinator (employees and contracted individuals only)

    Tips and Instructions

    • Click on "Create Individual Account" then click "next" (unless you already have a Keystone ID (username) and Password. If you do not remember either, create a new Individual Account).
    • Create a Keystone ID - the Keystone ID is actually going to be your username for your account.
    • You will be required to create 3 security questions and answers and provide an email address.
    • A temporary password will be sent to the email you provide.
    • In the body of the email you receive, click on "Child Welfare Portal". This will take you back to the login page.
    • Click on "Individual Login" and enter the Keystone ID you created and the temporary password from the email you received.
    • You will be prompted to answer several more questions and create a new password.
    • You will then be required to login again with your Keystone ID and the new password you created.
    • Answer all questions to create your "Clearance Application".
    • No authorization code is provided by the district, and you will need to enter debit/credit information for payment.
    • The system will ask if your orgainzation provided an authorization code, click "no" and complete your application.
    • It may take up to 14 days to receive the results of your clearance.
    • You will receive an email from the portal when your application has been processed. Use the Keystone ID and password you creaeted to log in as an "Individual Login" to get your results.
    • Keep a copy for your records an send the certificate to Tina Fair, Human Resources Coordinator (employee and contracted individuals only).

    Tips and Instructions

    • Register - Applicants must register with Identogo online and enter the following service code:  1KG6XN on the first screen. If you use an incorrect service code, it will not be accepted by the district. If you have any difficulties while registering online, you may call 1-844-321-2101 Monday through Friday from 8 a.m. to 6 p.m. EST. Registration is required before you proceed to a fingerprinting center. Fingerprinting locations can be found on the Identogo website when you register.  You will be required to pay with a debit/credit card when you register. No authorization code is available from the district.  
    • Schedule an Appointment for Fingerprinting - You will be prompted to schedule an appointment at a center of your choice. You may do a walk-in appointment, but these are processd on a fist come, first serve basis and you are still required to register for printing.
    • Confirmation Summary - Print the confirmation summary when you have completed your registration. Take the summary and your chosen photo ID document to the fingerprinting center.  
    • After Fingerprinting is Completed - Provide Tina Fair, Human Resources Coordinator with a copy of the confirmation summary or the Universal Enrollment ID "UEID" that is printed on the summary.  This ID will be used by the district to print an official clearance.  You will receive an unofficial clearance by postal mail from Identogo for your records.
