2016 1095's For Income Tax Purposes

This year you will receive two (2) 1095 forms for tax purposes.  One will be for district health insurance coverage with Capital Blue Cross for the 2016 period of January through June.  The second form will be for coverage with Highmark for the 2016 period of July through December. The 1095 forms are required to be in employee's hands by March 2 per the IRS.

These forms will come to you by postal mail and will be in separate envelopes.  Capital Blue Cross will submit for the first half of the year and Conrad Siegel Actuaries will send the forms on behalf of the district for the second half of the year.

1095 forms are not required per the IRS to file your taxes, they are simply required to be kept for you own records for auditing purposes.  However, some accountants may require you to provide the form before they will file your taxes for you.
